2025-2026 Forms Information Letter for Parents (pdf)

The forms provided below are to be filled out for the child that is attending Mayo Nursery School. This paperwork is due August 27, 2026. Your child can't start school until all paperwork is completed.
PARENT VOLUNTEER FORMS
The forms provided below are to be filled out for the parent(s) or guardian(s) that are going to volunteer in the classroom throughout the year. Atleast one adult per family must submit this paperwork by August 27, 2026.
The Release of Information form must be completed after June 1, 2026 so that it doesn't expire during the 2026-2027 school year. This form must be notarized, and a notary will be available at the August 27th meeting. If you would like to use our notary at the meeting, please fill out the form in advance but do not sign. The notary will check your Identification and witness your signature.
The other 3 requirements can be completed at any time.
